Dear Editor, A 45‐year‐old man, with an unremarkable dermatological or family history, presented with asymptomatic bluish‐brown cysts on the forehead and both cheeks since adolescence (a).1,2 The patient had a single 21‐year‐old son with similar facial lesions (b). Anatomical pathology testing revealed stratified epithelial cystic lesions with epidermal keratinization. The content was composed of sheets of keratin and pigmented terminal hair shafts with numerous melanin granules (c). In polariscopy, corticomedullary differentiation was noted (d). We proceeded to remove the cysts with a lancet after a superficial incision with a CO2 laser in superpulsed mode. After 10 months of follow‐up, no recurrences were noted (e). We present the first two cases of multiple pigmented follicular cysts with a familial association.
