Cholesterol is essential to the structure and organization of animal membranes and profoundly interferes with the involvement of membranes in cellular processes. With the unprecedented progress of biomedical nanotechnology in the past few decades, many studies have reported the introduction of cholesterol and its derivatives as part of carriers for drug delivery, as they enhance the pharmacokinetic behavior, cellular internalization, target specificity, and safety of the therapeutic agents. In this chapter, we first reviewed the common synthesis chemistry to obtain polymeric cholesterol derivatives. Next, the primary aim of this chapter is to discuss the design of different cholesterol-based drug carriers and their applications for disease treatment, mainly cancers.
