A primary intraosseous carcinoma (PIOC) is a type of SCC that develops from odontogenic cysts. The carcinoma is a fairly rare tumor that originates from odontogenic residual epithelium. This report is on a 59-year-old male who developed PIOC from a dentigerous cyst containing supernumerary impacted teeth. The patient had the chief complaint of nasal congestion and pain. Radiographic imagery depicted a supernumerary impacted teeth in the right anterior maxillary region surrounded by a cystic radiolucency. CT images revealed rough edges on the bones around the cystoid transmission, thus a biopsy was performed to confirm the diagnosis. The patient was diagnosed with PIOC as the biopsy showed that tumor cells with densely stained giant nucleis had invaded the connective tissues around the dentigerous cyst. A partial right maxillary resection and right neck dissection (Level Ⅰ-Ⅲ) were determined as the appropriate procedures for treatment. A maxillary prosthesis was created after surgery when the patient’s wounds were stabilized. No additional resection or adjuvant therapy has been performed. The patient has been followed-up closely for over 5 years and no evidence of recurrence or metastasis has been observed.
