The blood-retinal barrier (BRB) consists of inner (iBRB) and outer (oBRB) components and plays a crucial role in maintaining the retina's unique microenvironment. The inner BRB (iBRB), established by tight junctions (TJ) among endothelial cells, plays a crucial role in regulating selective permeability. Monocyte-derived microglia, pericytes, Müller cells, and astrocytes contribute to the dynamic nature of the iBRB, influencing immune responses. The outer BRB (oBRB) is formed by retinal pigment epithelial cells (RPE) sealed by TJ. Here, we review the BRB's relevance to retinal diseases, emphasizing the critical role of selective permeability. The chapter concludes with insights into clinical evaluation methodologies and the intricate interplay between BRB dysfunction, macular edema, and innovative treatment strategies.
