To the Editor: Extracorporeal photopheresis (ECP) is a therapeutic procedure currently used for immune modulation in lung transplant patients with chronic allograft rejection. During ECP, mononuclear cells are collected from patients' peripheral blood, incubated, and photoactivated with UV-A light before being reinfused.1 There are some concerns that ECP may increase risk for keratinocyte carcinomas (KC) as one of the agents used, 8-methoxypsoralen (8-MOP), is traditionally used for psoralen and ultraviolet-A (PUVA) phototherapy and has been shown to increase risk of basal cell carcinoma (BCC) and squamous cell carcinoma (SCC).
