A Complex Cause of Pleuritic Chest Pain

A 33-year-old man presented to the emergency department with pain in the right side of his chest. The pain had started 5 days earlier and was worse with deep inspiration and recumbency. He also noted mild swelling of his lower legs over the preceding several weeks.
