We report a case of a 61-year-old male patient who was referred to our emergency department with abrupt onset of severe chest pain lasting for the past 5 h. He had uncontrolled hypertension for 20 years and chronic smoking for 30 years. On admission, the physical examination revealed markedly elevated blood pressure of 180/110 mm Hg, regular pulse rate of 90 bpm and O2 saturation of 96% in the room air. No significant heart murmur was heard, and lungs were clear.
