• High prevalence of off-label psychotropic use was in adolescents with depression. • Off-label scenarios in high frequency are at a priority for digging evidence. • A significant gap is between limited evidence and pervasive off-label scenarios. • It is concerning to generalize adult use of psychotropic drugs to adolescence use.
