METHODS: We performed an analysis of prospectively collected data of consecutive patients undergoing radical cystectomy in 4 primary care Italian urology centers. All patients underwent detailed clinical history and physical examination, preoperative, perioperative and postoperative characteristics were recorded. Decision regret was evaluated with validated questionnaires. Satisfaction score ranges from 3 to 15 (Satisfaction=score<5)) while regret score ranges from 2 to 10 (Regret=score<5). Logistic regression analysis were used to evaluate predictors of regret and satisfaction after RC. RESULTS: One hundred and eleven patients with a median age of 68 years (60/70) were enrolled. 47/111 (42%) performed a robotic procedure. Overall 59/111 (53%) underwent a neobladder, 34/111 (30%) underwent ileal conduit and 18/111 (16%) ureterocutaneostomy. Median satisfaction score was 3 (3/4) while median regret score 8 (6/10). On logistic regression analysis there were no specific predictors of treatment satisfaction. On logistic regression analysis advanced disease (pT≥3) (OR=2,88;p<0,05), open surgery (OR=2,91;p<0,05) and adjuvant chemotherapy (5,53;p<0,05) are predictors of treatment regret. CONCLUSIONS: In patients undergoing surgery for bladder cancer treatment regret depends on stage of the disease and surgical approach. If confirmed in larger series the implementation of robotic surgery may improve the management and regret of patients undergoing radical cystectomy.
