Background: Pathophysiology of irritable bowel syndrome (IBS) is not fully known.Most patients relate their symptoms to food intake, which results in the restriction or elimination of many foods considered as symptom triggers.Restriction/elimination of food negatively affects their quality of life (QOL).Elimination diets as the FODMAP diet have proved efficacy specially in IBS with diarrhea.However, there are long-term safety concerns and difficulties to its implementation.A better dietary and nutritional approach is required in patients with IBS.Aim: Compare the outcome of two different dietary interventions on symptoms control and QOL of patients with IBS.Methods: Patients with IBS (ROME III criteria) were recruited at our GI motility lab from October 2016 to May 2018.Before randomization, patients were asked to fill a food diary and symptoms for one week.Patients randomly received two interventions for 4 weeks: a) recommendations of the guidelines of the National Institute of Health and Care of Excellence (NICE); and our dietary proposal, b) Specific food modifications according to individual tolerance and preferences based on the records in food diary.Abdominal pain, distension, constipation, diarrhea, were assessed in terms of frequency using a 4-point Likert scale (0 =none, 1 =mild, 2=moderate, 3=severe).Complete responder (CR) was defined if there was a decrease in ≥ 1 point from baseline in both, abdominal
