A Woman With Pyoderma Gangrenosum, Recurrent Abdominal Pain, and Hematochezia

Question: A 69-year-old woman with a recent hospital admission for diverticulitis presented with abdominal pain, cramping, and hematochezia. Past medical history included a diagnosis of pyoderma gangrenosum that was made 1 month prior after she had presented with a nonhealing cat bite. She also had a history of diverticulosis, hypertension, osteoarthritis, gastroesophageal reflux disease, and thyroid nodules. Her recent admission for diverticulitis occurred 3 weeks prior. It had manifested as abdominal pain in the left lower quadrant, nausea, anorexia, and diarrhea, with computed tomography (CT) imaging consistent with diverticulitis (Figure A) and improvement seen with administration of antibiotics.
