The Purkinje network has been well described as a source for ventricular fibrillation (VF) in patients with and without structural heart disease. The mechanistic understanding of the role of the Purkinje network is limited by the need for repeated mapping attempts. Waldmann et al (https://doi.org/10.1016/j.hrcr.2022.11.013) share a case of a 4-year-old girl with a history of nonobstructive hypertrophic cardiomyopathy (HCM). At the age of 9 she presented with a VF cardiac arrest during exercise and was successfully resuscitated.
