摘要
1091 Objectives: To analysis of 131I treatment effect and its influencing factors in patients with giant goiter and hyperthyroidism.Methods The total of 523 patients with giant goiter and hyperthyroidism who were treated with 131 I from May 2010 to May 2017 in the first hospital of Shanxi Medical University were retrospectively analyzed. We dynamically followed these patients for 1-10 years.Follow-up data included age, sex, course of disease, thyroid hormone and antibody levels, thyroid ultrasound, SPECT imaging, thyroid 131I uptake rate and 131I ablative dose. All patients were evaluated for efficacy. The efficacy was divided into: complete remission, hypothyroidism, partial remission, and ineffectiveness. T-test and chi-square test were employed to evaluate the date set. Logistic regression was used to analyze the factors affecting efficacy. Results (1)478 patients with effective follow-up ,the complete response rate was 35.4% (169 cases), the rate of hypothyroidism was 49.1% (235 cases), the partial response rate was 8.8% (42 cases), the ineffective rate was 4.4% (21 cases); the total clinical cure rate (including completeness relief and hypothyroidis ) were 86.8% (414 cases) and the effective rate was 95.6% (457 cases). (2)The lower the thyroid mass, the higher the incidence of hypothyroidism. (3) Factors associated with the efficacy of 131I included age, course of disease, thyroid quality, TRAb level, and 131I dose per gram of thyroid.Thyroid quality and 131I dose per gram of thyroid are the uppermost influencing factors among them.
Conclusions: 131I treatment of giant goiter with hyperthyroidism has the advantages of high total cure rate, small side effects, safe and effective.It is worthy of wide application in clinical practice.[Keywords] Giant goiter;Hyperthyroidism;131 I treatment; Curative effect
