Ortner syndrome was first reported by Norbert Ortner more than 100 years ago ( 1 Ortner N. Recurrenslahmung bei mitralstenose. Wein Klin Wschr. 1897; 10 (German): 753-755 Google Scholar ). He presented 3 cases of left recurrent laryngeal nerve (LRLN) palsy due to compression by the dilated left atrium secondary to severe mitral valve stenosis. Aortic intramural hematoma (IMH) rarely causes Ortner syndrome. Its most common symptom is chest or back pain. Herein, we present a rare case of painless aortic IMH presenting with Ortner syndrome. Written informed consent was obtained from the patient, and the study was approved by the institutional review board of Qilu Hospital, Shandong University.
