Intravenous thrombolysis and endovascular thrombectomy are the principal reperfusion therapies in patients with acute ischaemic stroke. Thrombolysis has predominantly been done with alteplase for over a quarter of a century, and tenecteplase has recently emerged as a suitable alternative after trial results showed non-inferiority versus alteplase.1 Although reperfusion therapies have substantially improved functional outcomes after acute ischaemic stroke, nearly half of patients still do not have favourable functional outcomes, and intracranial haemorrhage remains an important complication.
