One of the clinical dilemmas: the longer the more indicated?

We have read the article ‘Estimated incidence of previously undetected atrial fibrillation on a 14-day continuous electrocardiographic monitor and associated risk of stroke’ by McIntyre et al.1 The study shows that in patients over 65 years of age with hypertension and a pacemaker, the rate of atrial fibrillation (AF) detection over 6 min with 14-day electrocardiogram monitoring is 3.1%, and the stroke rate is approximately three times higher in these patients. In this study, the ehocardiographic examination was not unspecified, and it is unclear whether patients have a structural heart disease or heart failure. In fact, it was not stated that patients went to pacemaker implantation due to a conduction defect or implantable cardioverter-defibrillator implantation in heart failure with reduced ejection fraction.
