Two patients with paroxysmal hemifacial spasm were treated surgically with good result. In one of them the anterior inferior cerebellar artery was separated with lyophilized dura from the brain stem and from the pontine site of origin of the facial nerve. In the second case the whole facial nerve was dissected from its origin from the brain stem to the internal porus acusticus and lyophilized dura was inserted between the vestibular artery and the lateral part of the facial nerve.
