Objective
To explore the application effect of Family Participated Distance Nursing Mode in type 2 elderly diabetic patients.
Methods
A total of 120 type 2 elderly diabetic patients discharged from our hospital form December 2014 to March 2015 were selected as the study subjects and divided into the observation group and the control group according to the admission sequence number. The control group was given conventional regular telephone follow-up nursing mode and the observation group was given Family Participated Distance Management Mode. Each indicator, the self-control skill, awareness rate of diabetes knowledge, FPG and HbA1c of the two groups were compared.
Results
The score in the self-control skill, FPG and HbA1c of the observation group 12 weeks later were 58.42±4.87, (5.48 ± 1.44) mmol/L, (7.81 ± 1.20)%, and the score of the control group were 35.55 ± 6.21,(6.31 ± 1.92) mmol/L,(10.75±3.12)%. The difference was statistically significant (Z=-21.58,-7.24, -15.45, P <0.01). The FPG of the observation group had no change, but the control group changed significantly, (5.90±0.89) mmol/L before the intervention and (6.31 ± 1.92) mmol/L after the intervention (Z=-6.21, P<0.05).
Conclusions
Family Participated Distance Nursing Mode is conducive to improve the self-control skill, awareness rate of diabetes knowledge, FPG and HbA1c of elderly patients.
