A 47-year-old male building maintenance worker was injured by the connection hook on a lanyard. A, The anterior chamber image reveals lens subluxation with the vitreous herniating into the anterior chamber around the lens edge as observed on anterior segment OCT (B). The herniated vitreous touched the corneal endothelium. C, The vitreous hernia was barely visible on slit-lamp biomicroscopy image; however, it could be observed on the retro-illumination image captured on the slit-lamp (D).
