Cardiac troponin (cTn) released from damage myocytes. It is used as early marker for determination of myocardial damage we estimate correlation of troponin I level in patients with acute Myocardial infarction and its prognostic implication In heart failure and early mortality.An Observational Study was done in tertiary care center sample for plasma concentrations troponin I level analysed in 100 patients of acute myocardial infraction at 12 hours from symptoms. The 99th percentile value (0.04 ng/ml) was used to assess clinical accuracy for diagnosis of acute MI. Categerisations was done on their cTnI value baseline to ten times (Category 1), more than ten times to hundred times (Category 2), more than hundred times (category 3).The heart failure (P value 0.001) (Basis on Killip classification class 3 & 4) 13 patients out of 46 in category 2 and 32 patients out of 44 in category 3 were in failure during admissions or hospitalization and mortality rate 11% was significantly higher (P value 0.001) in 100 patients of acute myocardial infraction with increased (more than hundred times of upper reference range) troponin I level above the 99th percentile cTnI concentration.Increased level of troponin I (ten times to hundred times or more from the 99th percentile cTnI concentration) prognosticate risk stratification for adverse events like heart failure and early mortality in patients of acute myocardial infraction.
