Introduction: Recent insights into the pathogenesis of psoriasis have elucidated the pivotal role of IL-23/IL-17 axis. In particular, it was demonstrated that IL-23 is the “master regulatory cytokine” of the psoriatic inflammatory cascade. IL-23 inhibitors block “upstream” the IL-23/IL-17-axis and orchestrate the immunologic response toward impairment of pathogenic Th17. Herein, we aim to evaluate whether, in patients who failed an IL-17 inhibitor, continuing to target the IL-23/IL-17 pathway with IL-23 antagonists can lead to treatment response.
