Malignant melanoma can occur throughout the human body but testicular metastases are rare. We report on an 82-year-old man with stage IV cutaneous melanoma (Breslow thickness of 11 mm and Clark's level V) in the right upper arm. Combined FDG-PET/CT demonstrated widespread metastases involving subcutaneous tissue, muscles, lymph nodes, bones, pleura, lung, liver, and the right testis (SUV 12.6). After resection of the primary, the patient is now receiving systemic chemotherapy. PET is valuable in staging and management of patients with melanoma; PET/CT fusion clearly localizes lesions with abnormal FDG uptake and has improved accuracy over PET alone.
