McCullough,1993 A retrospective study of 217 consecutive primary total knee arthroplasties (TKA) was undertaken to identify factors that may adversely affect wound healing. Wound complications occurred in 9.5% of patients. Twenty-two percent of rheumatoid patients but only 4% of osteoarthritic patients suffered wound complications. Rheumatoid patients had a five-fold increased risk of developing wound healing problems compared with osteoarthrotic patients (p < 0.001). Age, ESR at the time of surgery, use of corticosteroids, pre-operative haemoglobin level, type of incision, duration of operation and several other factors were also analysed, and were found not to be significant. Patients with rheumatoid arthritis undergoing primary TKA are at increased risk of developing wound complications, and require extra care in tissue handling, haemostasis, and wound closure. |
